Telemedicine/Teledentistry Consults
We are pleased to able to offer Telemedicine Consults for NY Residents during the COVID-19 Pandemic. To start a consultation, please
complete the Telemedicine Consent Form (Click -Telemedicine Consent) and press on the light green link below.
We will try to bill your insurance whenever possible otherwise the fee for the consultation will be your responsibility.
Note: If you are a new patient you will need to have your patient registration completed prior to starting a consult with Dr. Ray.
