Financial Information (Effective October 1st, 2013)
For your convenience, we accept Visa, MasterCard, Discover and CareCredit. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Due to the constantly changing policies of insurance companies, we have established the following financial policies to help all patients understand their financial obligations.
Please remember as the patient, you are ultimately responsible for the fees for treatment.
- Patients with no insurance: The fee for treatment is due on the day of service. We will assist you in obtaining information about Care Credit (interest free dental credit plan) or you may use cash, check or a credit card. If you are interested in Care Credit you can also visit CARECREDIT.com for information and an application.
- Patients with insurance: We collect a pre-determined percentage of 30% on the day of service. We will then bill your insurance. For insurances we do not participate with, you will be reimbursed directly in some cases. Any balance due will be the patient’s responsibility. If you have a second insurance carrier you would like billed, it will be your responsibility to get us the Explanation of Benefits (if you were paid directly) so that we may bill your second insurance carrier for you.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to the actual procedure.
